Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
If you are taking regular medication you will have been issued a computerised repeat prescription request by the Doctor (this request slip is located on the right hand side of your prescription).
Please keep hold of your repeat slip in a safe place as this will save you having to fill out a medication request form at reception. This is our preferred method of request as this is a copy of your current repeat medication as detailed in your medical record.
To order further supplies of medication please ensure you have ticked all the items that you require on the slip as we will only process items that you have ticked.
This will ensure that we only issue medication that you require and avoid wastage.
Please note that any medication requested that does not appear on your repeat slip cannot be processed by the Administration Team and has to be dealt with by a Doctor.
How Do We Accept Prescription Requests?
- You may either post this slip to us or place it in the repeat prescription box situated in the waiting room or post it in the letter box situated in the porch.
- You may also fax your request to the surgery on 0844 815 1223
- If you would like us to post your prescription back to you please send us a stamped addressed envelope with your request and we will happily have your prescription returned to you in the envelope that you have provided.
- You are now able to order your repeat prescription via this website, This will involve a simple registration process. If you would like to access our Online Services please click on the right hand side where you will be directed to our online request form.
Please note you will be required to bring in a form of Identification either a Passport or Driving License before we can issue you with your username and password.
If you have lost or mislaid your repeat prescription slip, please print your request clearly on paper remembering to include the name and strength of the medication you require, your full name, address and date of birth. There are repeat prescription request forms available at reception.
For safety reasons we are unable to accept repeat prescription requests over the telephone but for URGENT occassions only where you have run out of medication we can get the doctor to ring you back the same day and the doctor will be able to take your request over the telephone.
